中鼻甲微波热凝加外折术治疗鼻睫神经痛86例

Microwave thermocoagulation plus outfolding of middle nasal concha for the treatment of 86 cases of nasociliary neuralgia

目的探讨中鼻甲微波热凝加外折术治疗鼻睫神经痛临床疗效和意义。方法对86例鼻睫神经痛患者,在局部麻醉前鼻镜和鼻内镜下先用微波热凝中鼻甲肥厚病变的粘膜,再将其骨折端外移,使中鼻甲不再与鼻中隔抵触,解除嗅裂狭窄。结果术后随访1年以上,中鼻甲形态均恢复正常,嗅裂宽敞,86例病例中均未发生嗅觉下降等并发症。治愈84例(97.7%),好转2例(2.3%),总有效率100%。结论中鼻甲微波热凝加外折术去除病变恢复中鼻甲生理功能,疗效显著,是治疗鼻睫神经痛的微创和理想的方法。

Objective To investigate significance and efficacy of microwave thermocoagnlation plus outfolding of middie nasal concha for treatment of nasociliary neuralgia. Methods Eighty-six patients with nasociliary neuralgia were performend with microwave thermocoagnlation ,which thermocoagnlated thickenging mucosa of middle nasal concha, and followed with outfolding, resulted in separation of middle nasal concha and nasal septum, reliefed olfactory cleft stricture. Results All cases were followed up for over one year , structure of middle nasal concha became normal, olfactory cleft became wide. There was no complication such as hyposmia. Among of 86 patients, 84 cases（97.7 % ） were cured, 2 cases（2.3 % ） were modified, total effective rate was 100 %. Conclusion Microwave thermocoagnlation plus outfolding of middle nasal concha is effective to remove the lesions and recover physical function of middle nasal concha, which is a microinvasive and ideal method for treatment of nasociliary neuralgia.
