十年连续内镜检出1747例大肠息肉分析

Analysis of anatomic distribution of 1,747 colorectal polyps over a period of 10 years

目的研究近十年大肠息肉发生部位的变化及与性别、发病年龄及病理类型的关系。方法分析1995～2004年我院内镜中心检出并经病理证实的1747例大肠息肉。息肉发生部位按右半结肠、横结肠、左半结肠、直肠及多发性息肉统计。比较前5年(1995～1999)和后5年(2000～2004)结肠息肉解剖部位的变化。结果1995～1999年检出大肠息肉共591例,以左半结肠、直肠为主(38.6%、25.9%),右半结肠、横结肠和多发性息肉分别为9%、7.8%和18.7%。200～2004年检出大肠息肉明显增加,为1156例。仍以左半结肠和直肠息肉多见(37%、24.6%),右半结肠、横结肠和多发性息肉分别为11.9%、10.2%和16.3%。前后5年相比,大肠息肉解剖部位无明显变化(P>0.05)。1747例结肠息肉中,男女之比为1.42:1;35～60岁年龄段发病比例高;病理类型以腺瘤性息肉为主(81.2%)。结论大肠息肉内镜检出例数增加迅速。近十年大肠息肉解剖部位无明显变化。息肉好发部位在左半结肠和直肠,以腺瘤性息肉为主。

Objective To investigate the change in anatomic distribution of colorectal polyps over a period of 10 years based on the consecutive endoscopic examination records. Methods The records of 1,747 final diagnosed colorectal polyps were reviewed. The location of colorectal polyps was divided into 4 groups： right-sided colon, transverse colon, left-sided colon, rectum and multiple. The distribution of polyps was compared between group year 1995 - 1999 and group year 2000 - 2004. Results During 1995 to 1999, the diagnosis of colorectal polyps was made in 591 patients. Most of polyps were in left-sided colon and rectum, accounting for 38.6% and 25.9%, respectively. Right-sided colon, transverse colon and multiplcpolyps accounted for 9%, 7.8% and 18.7%, respectively. During 2000 to 2004, the diagnosed polyps increased to 1,156 cases, but the distribution of polyps did not change dramatically （P 〉 0.05）： left-sided colon 37%, rectum 24.6%, right-sided colon 11.9%, transverse colon 10.2%, and multiple polyps 16.3%. 81.2% of the polyps were adenomas. Conclusion The number of polyps detected by colonoscopy increased rapidly in last five years, but the sub site of distribution did not change during last decade.
