摘要 目的探讨肺泡微石症(PAM)的CT特征。方法肺泡微石症3例,全部行常规CT,其中2例再行HRCT。回顾性分析其临床特征与CT表现。结果 CT特征性表现为:可明确双肺弥漫分布的微结节的钙化密度,以中下肺野的后胸膜、纵隔胸膜下及支气管血管束周围最密集,可形成“白描征”和“火焰征”。伴有不同程度的纤维化。HRCT则可进一步显示肺野毛玻璃样改变,小叶中心分布的微结节,小叶间隔及支气管血管束的钙化密度与增厚。结论肺泡微石症的CT表现具有特征性,结合临床特征可提示该病的诊断。 Objective To investigate CT features of pulmonary alveolar microlithiasis (PAM). Methods 3 cases of PAM underwent conventional thoracic CT scanning, while high-resolution CT was done in two cases. The clinical data and CT features were retrospectively analyzed. Results CT could show fine diffuse micronodular calcifications in both lungs, concentrating in the back and mediastinal subpleural zones of middle and lower lung fields and the areas around bronchlvascular bundles, forming flame sign and white line sign. All were associated with various degress of fibrosis. HRCT could further document the ground glass opacity in the lung, thickened interlobular septa as well as calcifications along the bronchovascular bundles. And the fine micronodulations in the center of the louble were also shown on HRCT. Conclusion PAM carries characteristic CT sings, which are very suggestive for making a correct diagnosis when clinical data are together taken into account.
