摘要
目的:探讨在寒区野外自然环境条件下创伤后再发冻伤的临床与组织学改变及自制“931”霜治疗效果.方法:对60只实验家兔采用随机分组、平行对照方法,观察创—冻伤的临床经过、组织学改变及组织愈合过程.结果:创—冻伤部位组织高度水肿,血管内附壁血栓形成,不同程度组织坏死,创面细菌感染率100%,创面愈合较对照组延迟6~8周(P<0.01),骨痂形成延迟,并易发生并发症.采用快速复温及局部“931”霜治疗,组织学改变明显减轻,创面感染率为26.67%,与未治组比较组织愈合缩短2~4周(P<0.01),骨折愈合缩短3~5周.结论:创伤后再发冻伤对机体损伤严重,及早采用快速复温并同时局部及全身治疗,可明显降低感染率,促进愈合,减少并发症.
Aim: To observe the clinical and histological changes in field experimental rabbit limb cold injury after wound in cold area, and the curative effect of “931” cream made by ourself. Methods: Observations on the clinical process, histological changes and healing of the wound cold injury in 60 rabbits were performed by random allocatiom and parallel control. Results: Serious edema and tissue necrosis of various degree in all layers, and mural thrombosis in vascular were found. Bacterial infection rate (BIR) was 100% on the surface of the wound, healing (vs, the normal group P < 0.01 ) was 6-8 weeks late, porosis was also put off; in such case, complications were likely to occur. After rapid rewarm and treatment with “931” cream local bandaging, histological changes were significant, BIR decreased 26.67%, healing time was 2-4 weeks earlier and porosis was moved up by 3-5 weeks, (vs, the untreated group, P <0.01). Conclusion: In organisms that were seriously damaged by cold injury after wound, rapid rewarming, local and systemic treatment, can reduce the BIR significantly, promote healing and decrease the complications.
