冷甘露醇静滴对高热患者降温的疗效比较 被引量:1

COMPARISON OF CURATIVE EFFECT OF COLD MINSTRA QUIET DRIP ON HIGH FEVER AND SUPER HIGH FEVER PATIENTS.

摘要 目的:探讨冷甘露醇商对高热和超高热患者降温的效果和机理。方法:应用4℃冷甘露醇静脉滴注对106例高热和超高热患者的降温进行随机单盲对照观察。结果:高热观察组显效(1 h内降温)占80.85%,有效(2 h内降温)占19.15%,总有效率100%,显著优于对照组(P<0.001)。超高热观察组显效75%,有效25%,总有效率100%,优于对照组(P<0.005)。结论:其方法简便,无毒副作用,降温同时能解除脑水肿,打断高热与脑水肿之间的恶性循环。其降温机理可能是:①直接降低血温。②高渗性利尿排出部分热量。③解除脑水肿,抑制中枢性高热。④清除自由基,有利于机体的康复。 Objective: To explore the effect on temperature dropping of cold minstra quiet drip for high for fever and super high fever patients and its mechanism. Method: The results of randomized single blind observation, which observed the application of 4℃ cold minstra quiet drip injection to 106 cases of high fever and super high fever children patients, were compared. Results: In figh fever observation group, more apparent effect rate (temperature dropping within 1 hour) was 80. 85%, effective treatment rate (temperature dropping within 2 hour) was 19.15 %, total, effective treatment rate (100 % ) was better than control group (P 〈0.001). In super high fever observation group apparent effect rate was 7596, effective treatment rate was 2596, total effective treatment rate (100 % ) was better than control group (P 〈 0.005 ). Conclusion: The method is simple and convenient, nonpoison effect. While dropping the temperature, it also could relieve brain dropsy, so as to break up the vacious cycle between high fever and brain dropsy. The mechanism of temperature dropping is probably that (1) Direct blood temperature dropping; (2) Releasing part of heat from high penetrating urine; (3) Relieving brain dropsy to stop central high fever; (4) Eliminating free base to facilitate recovery.
