The effect of sperm parameters on pregnancy outcome after intrauterine insemination

目的探讨处理前后的精液特征对IUI妊娠率的影响。方法收集分析342例不育不孕患者共372个IUI治疗周期,检测记录每个周期精液处理前后的各精液参数,并按照妊娠结果分成两组:妊娠组与非妊娠组,按照处理后精子活动总数(PTMS)分成3组:PTMS<10×106/ml,10×106/ml≤PTMS<20×106/ml,PTMS≥20×106/ml。比较各组间的精液参数和IUI妊娠率。结果372个IUI周期共获得62例妊娠,总周期妊娠率是16.67%,总病例妊娠率是18.13%。妊娠组与非妊娠组各精液参数无明显差别;PTMS<10×106/ml组获得的妊娠率明显低于后两组(P<0.05)。结论当PTMS>10×106/ml时,IUI治疗周期才有可能获得较为理想的妊娠率。

Objective To explore the effect of sperm parameters on pregnancy outcome after intrauterine insemination. Methods 324 infertile couples underwent 372 cycles of husband IUI. The semen parameters were recorded for each insemination. According to whether having a pregnancy or not, the couples were divided into 2 groups： pregnancy or nonpregnancy. Meanwhile the parameters and IUI outcome were compared among 3 stratifications of processed total motile sperm count （PTMS）： PTMS〈10×10^6/ml, 10×10^6/ml ≤ PTMS〈20×10^6/ml and PTMS ≥ 20×10^6/ml. Results Pregnancy occurred in 62 cycles, results in an overall pregnancy rate of 18.13%, and a cycle fecundity of 16.67%. No significant difference was found between pregnancy and nonpregnancy cycles. Moreover, the pregnancy rate was significantly lower for cycles with less than 10×10^6/ml PTMS. Conclusion Cycles with greater than 10×10^6/ml PTMS are significantly more likely to result in a pregnancy.
