上段食管癌切除后食管床内胃食管颈部吻合术

Esophagectomy and neck oner layer esophago-gastrostomy in the esophageal bed for the upper esophagus carcinoma

目的探讨上段食管癌切除后食管床内胃食管吻合术对术后并发症的影响．方法１９８７年５月－１９９４年５月，我院手术治疗上段食管癌１５８例，均采用食管床内胃食管颈部单层吻合．结果术后发生吻合口瘘１例（０．６％），心肺并发症２例（１．３％）．术后３ｍｏ～３ａ随访观察病人无进食后脑闷及呼吸困难。结论本术式的优点是可减少吻合口瘘，吻合口狭窄及术后心肺并发症的发生，尤其是时高龄及术前心肺功能差的病人更有利．

Objective. TO study the effects of esophago-gastrostomy in the esophageal bed on postoperative complications in pationts with resection of the upper esophagus carcinoma. Method One handred and fifty eight patients with upper esophageal carcinorna underwent esophagus resection with neck anastomosis in the esophageal bed from may, 1987 to may, 1994. Results There were 1 anastomostic leak (0. 06% ) and 2 cardiopulmonary complication (1 .3 %) in this series. The postoperative dyspnea and sensation of oppression in chest were not observed in following-up stage from 3 months to 3 years. Conclusion This procedure can reduce anastomostic leaks, anastomostic stenoses and the incidence of postoperative complication.Especially, it is more advantage to patients who were older and advanced cardiac and respiratory dysfunctions.
