对颈椎间盘突出症的再认识

The ossificatiou and calcification of the cervical ligamenta flava

报告经ＣＴ、ＭＲＩ及手术证实的颈椎间盘突出症２７例，着重讨论了颈椎间盘突出症与颈椎病的关系，认为颈椎间盘退变是两者的共同病理基础。颈椎间盘突出不仅是单纯的髓核突出，晚期还可伴有骨赘增生、椎管狭窄、ＯＰＬＬ等改变。作者认为，凡是与颈椎间盘突出有关的颈椎病变都应归入颈椎间盘突出症的范畴。

The ossification of ligamenta flava (OLF) or the calcification of ligamenta flava in cervical spine is relatively rarer, compared with that in thoracic and lumbar segments. 19 cases of OLF in Chinese were reported (6 cases were diagnosed pathologically, 13 cases were diagnosed according to operation and imaging finding). According to the review of these cases,it was found that cervical myolopathy was mostly causes by OLF in combination with the ossification of posterior longitudinal ligament (OPLL) or developmental cervical spinal canal stenosis ect., which was different from the lesion in thoracic area.The possible mechanism and clinical features of OLF, as well as its diagnosis and treatment methods were discussed.Author's address Department of Orthopaedics,The Third Teaching Hospital of Beijing Medical University, Beijing, 100083
