超声心动图诊断三尖瓣下移畸形

Echocardiography in diagnosis of ebstein′s malformation

目的探讨了超声心动图对三尖瓣下移畸形的诊断价值。方法应用彩色多普勒超声心动图技术诊断了7例三尖瓣下移畸形,注意观察三尖瓣隔叶与后叶的位置、三尖瓣的形态及活动度、房化右室的大小、三尖瓣返流程度、房间隔分流等改变,其中有3例经手术证实。结果7例中隔瓣下移畸形5例,后瓣下移畸形2例,三尖瓣前叶均发育冗长,房化右室明显增大,三尖瓣均有中度以上返流,合并继发孔型房缺3例,卵圆孔未闭1例。结论超声心动图对三尖瓣下移畸形诊断较为准确,能提供直观、可靠的依据,具有临床应用价值。

[Objective] To evaluate the value of echocardiography in diagnosing Ebstein＇s Malformation. [Methods] Color doppler echocardiography was performed in 7 cases. The position of the septal and posterior leaflets, the shape, movement of Tricuspid valve, the size of atrialized right ventricle, tricuspid reguritation, the branch flow of interatrial septum were paid attention to be observated by color doppler echocardiography. 3 cases were confirmed by operation. [Results] The downward displacement of the hinge points involved 5 cases of septal valve and 2 cases of posterior valve. All anterior leaflets were redundant. The atrialized right ventricle enlarged in all patients. Tricuspid regurgitation above moderate were in all patients. 2 patients in 3 cases with atrial septal defect and 1 patient with patent foramen ovale were confirmed by operation. [Conclusion] Diagnosis of ebstein＇s malformarion can be made by echocardiography which provides reliable information for clinic.
