三踝骨折伴下胫腓联合分离的手术治疗

Operative treatment of trimalleolar ankle fracture and abruption of lower tibiofibular ligament union by open reduction and internal fixation

目的探讨三踝骨折伴下胫腓联合分离手术治疗的疗效。方法对2000年10月-2004年6月的21例三踝骨折伴下胫腓联合分离的患者采取手术治疗进行回顾性分析。结果随访8-22个月,平均13.5个月,21例骨折均愈合。按Leeds等评定标准:优8例;良11例;差2例;优良率90.5%。结论三踝骨折伴下胫腓联合分离采用手术治疗可恢复并稳定踝穴和距骨的解剖关系,使踝关节早期功能锻炼,可获得满意的治疗效果。

Objective To explore the effects of operative treatment of trimalleolar ankle fracture and abruption of lower tibiofibular ligament union by open reduction and internal fixation. Methods The effect of 21 patients with trimalleolar ankle fracture and abmption of lower tibiofibular ligament union for operative treatment from October 2000 to June 2004 were analyzed. Results The follow - up, ranging from 8 to 22 months （an average of II. 5 months）, showed that good trearment was obtained in all the eases. According to Leeds＇s standard of ankle joint, excellent ：8 cases： good ： 11 cases ; poor ：2 cases. The total percentage of good to excellent clinical results was 90.5 %. Conclusion Operative treatment of trimalleolar ankle fracture and abruption of lower tibiofibular ligament union by open reduction and internal fixation has satisfactory results with anatomic reduction, stability and finn fixation. It helps the patients to practise early exercises of ankle joints.
