Amy、Lips和U-trypsinogen-2测定在诊断AP中的应用 被引量:1

摘要 目的评价淀粉酶、脂肪酶和尿胰蛋白酶原-2诊断急性胰腺炎的价值。方法检测因急性腹痛而住院的病人血、尿淀粉酶(Amy)、血脂肪酶(Lips)及尿胰蛋白酶原-2(U-trypsinogen-2)共72例,其中急性胰腺炎(AP)22例,非急性胰腺炎组50例。观察尿胰蛋白酶原-2阳性持续时间。结果22例AP血、尿标本Amy阳性分别为20和18例;Lips、U-trypsinogen-2阳性均为19例,血、尿Amy敏感性、特异性和准确度分别为90·9%、88%、89%和81·8%、90%、88%;Lips分别为86·3%、96%、93%;U-trypsinogen-2分别为86·3%、98%、94%。尿胰蛋白酶原-2阳性持续时间可达15d。结论尿胰蛋白酶原-2在诊断单纯性AP时,其敏感性和特异性与Amy、Lips相近;而在早期检出、尿中持续阳性时间以及在诊断急性出血性(重症)胰腺炎则优于Amy和Lips。
