摘要
目的探讨肝硬化门脉高压患者门静脉血流动力学指标与自由门静脉压(FPP)之间的公式转换。方法我科自1998年6月~2004年6月对45例肝硬化门脉高压患者用彩色多普勒测量门静脉直径(PD)和门静脉平均血流速度(PVa)并术中测量FPP。利用流体动力学和高等数学的知识建立门静脉血流动力学模型,推导出血流动力学指标转换为FPP的数学公式(简称:朱氏转换公式)。利用朱氏转换公式计算的FPP与术中测量FPP相比较。结果术前彩色多普勒测定的PD、PVa代入朱氏转换公式计算出FPP与术中测量FPP比较无显著差异(P>0.05)。结论朱氏转换公式利用彩色多普勒测量的PD、PVa计算FPP是一个快速、可靠、便捷的方法,对临床动态监测FPP有非常重要意义。
Objective To explore the formulary conversion of diameter of the portal vein (PD)and average speed of portal vein (PVa) into free portal vein pressure (FPP) in patient with cirrhosis by mathematical methods. Methods From June 1998 to June 2004 PD as well as PVa were examined in 45 patients with cirrhosis used and FPP was measured during the operation. color Doppler ultrasonography before operation, The model of portal vein hemodynamics was established through higher mathematics and hydrohemodynamics, and formula was deduced from the model. FPP calculated by preoperative Zhu's formula to FPP measured during the operation were compared. Results No statistical difference was observed between calculated FPP by Zhu's formula and measured FPP during operation. Conclusion FPP converted from PD and PVa by Zhu's formula is a reliable,prompt and simple method, and it may be useful in dynamic monitoring FPP.
