鼻咽癌高剂量率近距离治疗的近期疗效观察

Observation on Short-term Curative Effect of HDR Brachytherapy for Nasopharyngeal Carcinoma

作者对４８例首次治疗的鼻咽癌患者采用外照射结合腔内高剂量率近距离治疗。其中计划性内外照射４４例，外照射后补量４例。腔内治疗使用￣（１９２）Ｉｒ放射源，剂量参考点设在距放射源中心轴０．８～１．５ｃｍ处。每次参考点剂量６～８Ｇｙ，共１～３次。外照射剂量为６０～８０Ｇｙ。治疗结束时鼻咽癌局部控制率９３．８％。１年和２年生存率分别为９７．８％和９５．５％。作者结合文献对腔内治疗的适应证及内外照射的剂量匹配进行探讨。

new cases of nasopharyngeal carcinoma were treated by external irradiation combined with intracavitary HDR brachytherapy,Of these, intracavitary irradiation combined with external irradiation was given on plan initially in 44 cases,as a supplementary treatment after external irradiation, intracavitary irradiation was given in 4 cases.￣(192)Ir as radioactive source was used ln brachytherapy,and the point at 0.8￣1. 5 cm from the central axis of source was used as a dose reference point. The reference point dose ranging from 6 Gy￣8Gy each time and a total of l￣3times was given.The dose of external irradiation was 60 Gy￣89 Gy.The results showed that the local control rate after completion of treatment was 93.8%, the l year and 2 year survival rate were 97. 8%and 95.5% respectively.The indication of HDR brachytherapy for nasopharyngeal carcinoma and the dose match of external and intracavitary irradiation were discussed.
