外固定器在严重多发伤救治中的应用

The Application of External Fixators in Treatment of Severe Multiple Trauma

目的:探讨应用骨外固定治疗严重多发伤骨折的方法及特点。方法:自1992年开始对严重多发伤骨折的37例患者在快速复苏和处理威胁生命的脏器伤同时,积极采用单侧外固定器迅速进行四肢骨折固定。对于火器伤及开放粉碎骨折不能Ⅰ期闭合伤口时,用骨外固定器固定骨折,便于伤口的处理。结果:除2例因多器官衰竭死亡外,其余35例均痊愈,骨折全部愈合,肢体功能良好。结论:在严重多发伤骨折的救治中,早期应用骨外固定器固定骨折可减少出血休克、脂肪栓塞等并发症,为其他器官创伤的救治创造条件,并有利于患肢骨愈合和功能恢复。

Aim:To evaluate the clinical results of external fixation in the trea(?)ent of severe multiple trauma. Methods:Since 1992,37 patients with multiple fractures have been treated with unilateral external fixators for fractured extremities,at the same time when rapid resuscitation and urgent management of injured vital organs were performed. For gunshot fracture or open fractures,external fixators seemed to be the first choice because of easy wound manage- ment.Results:Thirty-five patients were cured with good functional results except that two patients died of MOF.Con- clusion:In the treatment of multiple trauma,immediate use of external fixators can reduce the complications of hemor- rhage shock and fat embolism,establish the possibility of the management of other organ trauma,and be helpful for the healing and functional recovery of the injured extremities.
