摘要 目的探讨缺氧诱导因子-1α在宫颈癌中的表达及意义。方法采用免疫组化方法,回顾性研究109例宫颈癌中HIF-1α的表达,并与10例正常宫颈组织、16例宫颈上皮内瘤变作同步对比。结果正常宫颈、宫颈上皮内瘤变、宫颈原位癌、宫颈浸润癌组织中HIF-1α的阳性表达率分别为0%、12.5%、43.5%、81.4%。宫颈原位癌和宫颈浸润癌组织中HIF-1α表达显著高于正常宫颈和宫颈上皮内瘤变(P<0.01)。在不同组织学分级(即高、中、低分化)的宫颈浸润癌组织中,HIF-1α的阳性表达率逐渐增高,与组织学分化程度呈负相关(r=-0.769,P<0.01);临床分期Ⅲ期宫颈癌中HIF-1α的表达明显高于Ⅰ期,Ⅳ期明显高于Ⅱ期(P<0.01);但不同病理类型宫颈癌组织中HIF-1α的表达差异无统计学意义(P>0.05)。结论HIF-1α是宫颈癌形成的早期事件,其水平升高可能导致肿瘤向恶性表型发展,HIF-1α有可能成为新的早期筛查和判断宫颈癌生物学行为的诊断指标。 Objective To study the expression of hypoxia inducible factor-1α(HIF-1α)protein in cervical cancer and its biological signicificance. Methods The expression of HIF-1α was assayed by means of immunohistochemical technique in 109 cervical cancer , 16 cervical intraepithelial neoplasia(CIN) and 10 normal cervical tissue specimens. Results The expression rates of HIF-1αprotein were 0%, 12.5%,43.5%, 81.4% respectively in normal cervical tissue, CIN, cervical carcinoma in situ and invasive carcinoma of cervix uteri. The positive rate of HIF-1α in cervical carcinoma in situ and invasive carcinoma of cervix uteri was significantly higher than that in nomal cervical tissue and CIN(P〈0.01 ). There was positive correlated between HIF-1α negative expression and histological differentiation(r=-0. 769 P〈0.01) . The marked difference existed between grade Ⅱ and grade Ⅳ(P〈0.01). But statistical analysis revealed that the expression of HIF-1αwas not relation to tumor histological subtypes(P〉0.05). Conclusion Overexpression of HIF-1α is the preliminary incident of the formation of cervical cancer, which may induce carcinoma into malignant phenotype. Thus it may be the early diagnostic parameter and be used as a biological mark of cervical cancer.
