糖尿病患者行胆囊手术的围术期处理

Treatment in perioperative period of gallbladder in diabetic patients

目的探讨胆道结石合并糖尿病的患者行胆囊手术的围术期处理。方法胆道结石并糖尿病患者86例,拟行胆囊摘除手术治疗。将病人分为血糖控制组(注射胰岛素,严格控制血糖在5.6mmol/L以下)和非控制组(注射胰岛素,血糖控制在5.6￣10.0mmol/L之间)各43例。结果血糖非控制组,手术治愈出院40例,死亡3例,切口感染者4例,肺部感染3例,尿路感染5例,胆瘘1例,死亡原因为糖尿病高渗性昏迷、术后大面积脑梗死和急性心肌梗死各1例:血糖控制组,43例中仅肺部感染3例,但均全愈出院。结论糖尿病患者进行胆囊手术,围手术期保持血糖正常稳定,是手术成功的关键。

【Objective】 To study The treatment in the perioperative period of gallbladder in diabetic patients. 【Methods】 86 diabetic patients under the operation of gallbladder were divided into control group （Blood sugar was treated by insulin under 5.6 mmol/L） and uncontrol group. 【Results】 There were 3 eases died and 12 eases developed infection in perioperative in uncontrol group. There was no case died and only 3 cases developed infection in control group. 【Conclusion】 In diabetic patients, it is a key point to treat with insulin and keep the blood sugar normal in the perioperative period of gallbladder.
