细针吸引细胞学检查在乳腺肿块诊断中的应用

目的探讨细针吸引细胞学检查对乳腺肿块良恶性的诊断及鉴别诊断。方法总结528例乳腺肿块行针吸细胞检查166例,与其肿块的切除病理对照结果相比较。结果该组166例中恶性94例,良性72例。针吸的假阴性率8.5%,去除未吸出上皮细胞的4例后,纠正假阴性率4.3%,假阳性率2.8%,其敏感性为91.5%,特异性为97.2%,阳性的可预示性为97.7%。结论细针吸引细胞学检查对乳腺肿块良恶性的鉴别上提供了可靠的依据,尤其对鉴别乳腺癌和浆细胞性乳腺炎有帮助。

【Objective】 To investigate diagnosis of breast mass by fine needle aspiration, that differentiate benign or malignant mass. 【Methods】 From Jannary 1991 to October 2003, 528 breast mass by FNA cytologic examination was summarized. 166/528 breast mass were excised and pathologically compared with cytology. 【Results】 Of 166 excised patients, 94 ale malignant, 72 arc begnign. False negitive rate is 8.5%, excepting 4 cases aspirated satisfactorily, the correct false rate is 4.3%. The false positive is 2.8%. It＇s serlsitivity is 91.5%. The specificity is 97.2%. The positive predictive value is 97.7%. 【Conclusion】 FNA is usefule for differentiating benign and maglignant mass, especially distinguishing cancer from plasma cell mastitis.
