大网膜移植治疗腕部严重电击伤 被引量:1

摘要 本文介绍1例10kV电击伤,电流出入口分别在右腕和右上腹的病例,伤后6天给以扩创:腕部血管损伤,腹部腹膜发黑,开腹将大网膜带蒂游高出腹腔,通过皮下隧道,部分覆盖在右上腹;一部分再通过皮下隧道到腕部,用胃网膜右动脉与挠动脉远端吻合,外用髂腹股沟轴形皮瓣将腕部创面封闭。术后23天皮瓣断蒂的同时,把胃网膜动脉的近端与挠动脉近端吻合,恢复和重建手的血供,从而保留手和手的功能。本研究目的是利用大网膜的组织活动度大,能分泌一些活性酶,血循环丰富且是轴形血管的特点,用来治疗无其它方法可用的腕部电击伤。通过1例临床成功的经验,进行探讨研究。 This paper introudces that a case was burned by 10000 V.The entrance forthe electricity to come into is in right wrist and the exit is in right upper abdomen.Six daysafter injury,the patient was given debridement:blood vessel of the wrist was severly injured,the peritoneum was dark,the great omentum was freed with the abdomen cut,passingthrough a subcutaneous tunnel,a part of omentum covered up right upper abdomen and theother omentum covered up the wrist via tunnel again.The gastroepiploic right artery wasused to anastomose with far extremity of the radial artery and then using groin axis-shapedskin flap closed up the wound.While cutting off pedide,the close end of gastroepiploic rightartery was used to anastomose with the near extremity of radial artery to recover and recon-struct blood circulation of the hand.The omentum has a lot of tissues and is easily trans-fered;it has a well-known blood vessel,abandant blood circulation;it can secret some orga-nized enzymes and its blood vessel shapes axis. All of this characteristics can be used to treatsevere electrical burn on the wrist,and we can inquire into it through this paper.
