超声引导下鹿角状肾结石的手术治疗

Ultrasonography-guided renal parenchyma incision for patients with staghorn-like renal calculi

目的：分析术中超声在鹿角状肾结石手术中的价值。方法：对21例鹿角状肾结石患者在超声引导下行肾切开取石术。结果：本组病例均一次取石成功，平均手术时间120min，术中需输血者2例，输血量200～400ml，术后6个月复查B超，其中1例出现直径0．8cm术侧肾内结石。结论：术中超声辅助行肾切开取石术效果满意、安全、可靠、易掌握，对肾功影响小。

Objective：To evaluate the efficacy of uhrasonography - guided renal parenehyma incision for patients with staghorn -like renal calculi. Methods： A retrospective analysis was done for 21 patients with staghorn -like calculi treated with renal parenehyma incision under ultrasound guidance. Results： Renal parenchyma incision was successfully performed in all cases, with a mean operative time of 120 min, 2 cases needed intraoperative blood transfusion which ranged from 200 to 400 ml, and the mean blood loss wgs 150 ml. During a follow - up period of 6 months, a 0.8 em residual kidney stone was found in one patient, and no calculus in the other patients. Conclusion：This procedure has the advantages of salty, high clearence rate, slight impairment of renal function, and is indicated for the patients with staghorn- like calculi.
