五羟色胺在胃肠道功能性疾病中的研究现状 被引量:59

摘要 五羟色胺(5-HT)是参与调节胃肠道运动和分泌功能的重要神经递质,95%来源于肠道黏膜层的嗜铬细胞.胃肠道主要有5-HT1、5-HT2、5-HT3、5-HT4、5-HT7受体.5-HT在发挥生理作用后通过肠黏膜上皮细胞膜上的5-HT转运体(SERT)再摄取至细胞内灭活.SERT基因5’启动子区存在多态性位点5-HTTLPR,形成L/L、L/S、S/S三种等位基因类型,S型较L型转录效率降低.胃肠道通过肠神经系统自主、协调地完成蠕动、分泌反射.肠嗜铬细胞释放5-HT,激活黏膜下传入神经纤维,通过肠神经系统,调节局部的兴奋和抑制.5-HT信号系统异常可导致胃肠道动力及分泌功能异常、内脏高敏感性,与慢性便秘、肠易激综合征、腹泻及功能性消化不良等胃肠道功能性疾病密切相关.对SERT表达在肠易激综合征发病机制中作用的研究是目前的热点.SERT基因多态性与肠易激综合征各型间可能存在联系.
