鼻空肠营养管行肠内营养治疗胃瘫八例报告

Enteral nutrition by naso-jejunal nutrition duct for gastroparesis in 8 cases

回顾性分析胃镜辅助下,放置鼻空肠营养管行营养支持治疗胃大部切除术后,胃瘫综合征(postsurgicalgastroparesissyndrome,PGS)8例患者的临床病理资料,结果显示,本组病例PGS发生率为3.02%(8/265),一般发生于术后4～7d,胃肠毕Ⅱ式吻合患者的发病率较高。PGS的发生是由多种因素诱发的,上消化道造影及胃镜检查是诊断PGS的可靠方法,治疗上宜采用非手术疗法,避免手术治疗,早期肠内营养,特别是胃镜辅助下放置鼻空肠营养管行营养支持是治疗胃瘫的一个非常有效的措施。

We analysed retrospectively the clinical data of 8 patients with postsurgical gastroparesis syndrome （PGS） after subtotal gastrectomy, infused the enteral nutrition preparation by naso-jejunal nutrition duct intubation under the guidance of gastroscope. The results showed that there were 8 patients suffering from PGS （3.02% in total 265 patients）,occurring within 4-7 d after operation. （3astroparesis was more frequent in those patiens who after a Billroth Ⅱ type gastrojejunostomy. PGS is induced by multiple factors. Radiography of upper gastrointestinal tract and gastroscopy are main methods to diagnose PGS. The methods of nonoperation are effective to patients with PGS, and operation should be avoided. Early enteral nutrition is effective means for treating PGS by naso jejunal nutrition duct intuhation under the guidance of gastroscopc.
