广西壮族地区居民胃癌发病高危因素初探

目的探讨壮族居民胃癌流行病学特征，并找出可以干预的一些发病因素。方法以2001年1月至2005年6月经胃镜检出及病理学检查确诊的胃癌131例作为研究对象，检测胃幽门螺杆菌（Hp），进行问卷调查，按1：2配对抽查健康体检262例做对照组，进行条件logistic回归分析。结果胃癌胃镜检出率1．38％（131／9485）；男：女为2．85：1，年龄17～83岁，平均67．45岁，50～70岁为胃癌高发年龄段；癌变部位由高到低依次为胃窦、胃体、贲门、胃底，病理学分型以腺癌为主。胃癌组Hp检出率为58．78％。胃癌相关的几个主要因素是：Hp感染、饮用井水／池塘／蓄存雨水、水源缺乏居住地、常食用高盐腌制腊肉、常食用蔬菜果品等（OR分别等于29．184、18．307、5．443、5．438、0．628，P＜0．05）。结论本地壮族居民具有其独特流行病学特征。改善生活居住条件特别是饮用水状况和改变不良饮食习惯可能是降低胃癌发病的有效干预措施。

Objective To investigate the epidemiologic characteristics of gastric cancer of Zhuang nationality residents , and to search for methods to deal with them. Methods 131 cases from Jan. 2001 to Jun. 2005 with gastric cancer were chosen as treatmnet group, they were diagnosed by gastroscope and Pathology examination and were ex- amed the infection of Helicobacter pylori（HP） and tested by questionnaire. 262 normal people were chosen as control group according to the proportion of 1/2. they wereanalysed by the logistic regression analysis. Results gastric cancer detectable rate of gastroscopes was 1.38% （131/9485）, the proportion of which between males and females was 2.85/ 1 ,aging from 17 to 83,with an average age of 67.45 years. People aging from 50-70 was high risk popuration . the region had a higher incidence rate were Pyloric part , body of stomach, cardiac orifice in turn. adenocarcinoma was the main pathology. The detectable rate of HP was 58.78% in treatment group, the factors related to gastric cancer included： infection of Hp; drinking the well water, pool water or stored rainwater; oligohydria; having preserved ham bloated in salt as well as vegetables and fruits（OR was 29. 184,18. 307,5. 443,5. 438,0. 628respectively, P〈0.05）. Conclusion The native Zhuang nationality residents have their specialpidemiologic characteristics. To improve the living conditions of the residents , especially of their drinking water, and change bad eating habit would be effective to reducemnet of the chances of suffering from gastric cancer.
