摘要 目的:探讨非酒精性脂肪肝患者(nonalcoholic fatty liver d isease,NAFLD)皮下、大网膜脂肪组织瘦素mRNA表达水平与血浆瘦素、三酰甘油、体重指数、腰臀比(WHR)的相关性。方法:用SYBR G reenⅠ实时定量RT-PCR方法检测NAFLD患者脂肪组织瘦素mRNA的表达水平,用ELISA方法测定血浆瘦素水平。结果:NAFLD各组皮下及网膜脂肪组织瘦素基因表达和血浆瘦素浓度较对照各组显著升高(P<0.05),瘦素基因表达和血浆瘦素浓度(r=0.69,P<0.05)与三酰甘油(r=0.32,P<0.05)、体重指数(r=0.57,P<0.05)、腰臀比(r=0.50,P<0.05)相关。结论:肥胖和非肥胖的NAFLD患者存在高瘦素血症及脂肪组织瘦素mRNA高表达,提示NAFLD患者存在瘦素抵抗。 Objective: To study the leptin mRNA expression in subcutaneous and omental adipose tissues of nonalcoholic fatty liver diseases ( NAFLD ) and their relation with blood leptin, triglyceride, total cholesterol, blood glucose, body mass index and waist hip ratio. Methods: Leptin mRNA expression levels in adipose tissue were measured by SYBR Green I quantitative real-time PCR. The level of plasma leptin was measured with ELISA kit. Results : The relative mRNA expression of leptin and blood leptin in NAFLD differed significantly from those of controls (P 〈0. 05 ). The expression levels of subcutaneous and omental adipose leptin mRNA in NAFLD were positively correlated with blood leptin ( r = 0. 69, P 〈 0.05 ), triglyceride ( r = 0. 32, P 〈 0.05 ), body mass index ( r = 0. 57, P 〈 0.05 ) and waist hip ratio( r = 0. 50, P 〈 0.05. Conclusion : The obesity and nonobesity patients with NAFLD showed high levels of blood leptin and the leptin mRNA expression in adipose tissues. These findings suggested a leptin resistance in patients with NAFLD.
