腓肠神经逆行岛状皮瓣联合骨外支架治疗胫骨开放性骨折

The Treatment of Open Tibial Fractures Using Distally-based Island Flap Pedicled with Sural Nerve and External Fixation Apparatus

目的:探讨腓肠神经逆行岛状皮瓣联合外支架治疗胫骨开放性骨折的手术方法及临床疗效。方法:18例胫骨开放性骨折患者,清创后应用外支架固定,Ⅱ期行腓肠神经逆行岛状皮瓣闭合伤口。结果:18例均皮瓣成活,2例部分皮肤坏死经换药后治愈。15例骨折临床愈合,3例需行髓内钉固定治疗。结论:腓肠神经逆行岛状皮瓣联合外支架治疗胫骨开放性骨折具有较好临床效果。

Objective ：To discuss the operation and clinical effect of the treatment of open tibial fractures with the sural nerve pedicled island flap and external fixation apparatus. Methods： External apparatus was used to fix after debridement and the wound closured in secondary operation by sural nerve pedicled island flap in 18 cases of open tibial fractures. Results： 18 case＇s flap survived. Partial skin necrosis occurred in 2 cases and then healed with debridement and dressing change. 15 fractures healed, and the other 3 were treated using the intramedullary interlocking nails. Conclusion ： treatment of open tibial fractures with the sural nerve pedicled island flap and external fixation apparatus showed desirable outcomes in the patients.
