胸膜凹陷征在肺外周结节CT诊断中的价值

Value of pleural indentation in CT diagnosis of peripheral pulmonary nodules

目的探讨有胸膜凹陷征(p leural indentation,PI)的肺外周结节的CT诊断,提高对胸膜凹陷征及其相关征象的认识。方法对经手术切除或经皮肺穿刺活检病理证实的58例有PI的肺外周结节的CT表现与病理结果进行回顾性分析。结果58例肺外周结节中,周围型肺癌47例,良性结节11例。Ⅰ、Ⅳ、Ⅴ型PI及胸膜凹陷相关结节切迹征(nod-u le notch due to p leural indentation,NNPI)多为周围型肺癌的CT表现;Ⅱ、Ⅲ型PI多见于结核球等良性病变。结论PI分型在肺外周结节的诊断与鉴别诊断中有重要价值。

Objective To study the prognostic effects of pleural indentation （PI） in CT diagnosis of peripheral pulmonary nodules in order to enhance the knowledge of pleural indentation. Methods The characteristics of pleural indentation in CT image of 58 cases of peripheral pulmonary nodules were analyzed retrospectively and its relation with the surgical and pathological results. Results In 58 cases of peripheral pulmonary nodules, 47 were peripheral lung cancer, 11 were benign nodules. PI- Ⅰ , PI-Ⅳ, PI-Ⅴ and nodule notch due to pleural indentation （NNPI） were seen more often in peripheral lung cancer, while PI-Ⅱ and PI-Ⅲ more observed in tuberculoma. Conclusion PI-typing can contribute to the correct diagnosis of peripheral were pulmonary nodules.
