摘要
目的探讨骨瓣漂浮结合硬脑膜减张修复在颅脑手术中的应用。方法术中硬脑膜减张严密修复,让骨瓣漂浮于骨窗硬脑膜上。结果硬脑膜减张修复及骨瓣漂浮术后早期有颅内减压作用,待脑水肿消退后,骨瓣自动回复,后期骨瓣可达临床愈合,颅脑手术并发症明显减少。结论颅脑手术治疗中,行骨瓣漂浮结合硬脑膜减张修复具有可靠地自动减压作用,避免骨瓣及硬脑膜缺损的并发症,免去二期颅骨成形术给病人带来的痛苦和经济负担。
Objective To explore the clinical application of floating cranial bone flap combined with tension-reduced pachymeninx repair in the craniocerebral operations. Methods During the operation, the cranial flap were kept floating on the pachymeninx of bone window repaired carefully. Results Floating flap could reduce intracerebral pressure in the early time and the bone flap would retrieve its position automatically after the subsidence of cerebral edema. The cerebral flap got clinically recovered at last. Conclusion The application of floating cranial bone flap and tension-reduced pachymeninx repair in neurosurgery is a reliable way to decrease intracerebral pressure and in the mean time it will abate the suffering and expenditure of the patients by a secondary cranial bone repair that has to be taken for cranial bone defection.
