Arnold-Chiari畸形并脊髓空洞症125例手术治疗疗效评价

The surgical treatments and curative effects evaluation of Arnold-Chiari malformation complicated with syringomye- lia: a study of 125 patients

目的探讨ArnoldChiari畸形并脊髓空洞症手术方式及术后疗效。方法对125例MR证实的ArnoldChiari畸形并脊髓空洞症患者的手术治疗情况进行回顾性分析。患者分别行后颅窝减压及枕大池扩大重建。结果出院时患者症状消失和改善共110例,有效率88.0%。随访114例患者,98例复查MR,枕大池解剖成形者87例,脊髓空洞缩小68例,远期有效率77.2%。结论后颅窝减压及枕大池扩大重建是治疗ArnoldChiari畸形并脊髓空洞症的有效方法。

Objective To discuss the surgical treatment and curative effect of Arnold-Chiari malformation complicated with syringomyelia. Methods The surgical treatments of 125 cases were analyzed retrospectively. They were performed by posterior fossa decompression, reconstruction of cisternals. Results The clinical signs and symptoms had been eliminated and improved in 110 cases （88.0%） until patients were discharged from hospital. 114 cases（77. 2% ） were followed up from 6 months to 10 years. The MR findings in 98 patients demonstrated that the cisternals reconstructed and expanded in 87 cases, the cavities in spinal cords reduced in 68 cases. Conclusions Posterior fossa decompression, reconstruction of cisteruals should be effective methods for treament of Arnold-Chiari malformation complicated with syringomyelia.
