急性肺动脉栓塞的介入治疗

Interventional treatment for acute pulmonary embolism

目的探讨介入技术在治疗下肢深静脉血栓致急性肺动脉栓塞中的临床疗效。方法对32例下肢深静脉血栓致急性肺动脉栓塞患者实施经皮穿刺下腔静脉滤器置入术和肺动脉碎栓、局部置管溶栓治疗，以开通肺动脉。结果滤器和导管均成功安全置入，经治疗后临床症状明显改善，并发脑出血1例，死亡1例；31例患者随访5～48个月，无再发肺动脉栓塞。结论下腔静脉滤器置入和经导管栓子祛除术是预防和治疗下肢深静脉血栓致急性肺动脉栓塞安全、有效的方法。

Objective To evaluate the efficacy of interventional therapy in acute pulmonary embolism （PE） with deep venous thrombosis（DVT）.Methods 32 PE patients with DVT were treated with inferior vena eava filter placement, thrombus fragmentation and transcatheter local fibrinolysis thrombolysis, Results After placement of filter and catheter, most patients showed significant clinical improvements but one case died of cerebral hemorrhage,31 cases were followed up for 5 to 48 months and none of recurrent PE was found, Conclusion Inferior vena cava filter placement and transcatheter embolectomy are safe and effective approaches for preventing and treating acute pulmonary embolism induced by DVT.
