^(99m)Tc-HL91显像对乳腺癌的诊断价值

摘要 目的:探讨99mTc-HL91乏氧显像对乳腺癌的鉴别诊断的临床价值。方法:51例乳腺包块患者行99mTc-HL91早期(1h)、延迟(4h)平面显像和延迟(4h)断层融合显像,利用计算机感兴趣区(ROI)技术,计算靶/非靶比值(T/TN)。结果:恶性组在1h、4h平面和4h断层图像上T/TN值分别为1.68±0.16、1.85±0.23、2.28±0.49,良性组为1.15±0.18、1.23±0.25、1.43±0.37,良恶性组之间均有显著的差异性(P<0.01);恶性组4h平面T/TN值明显高于1h T/TN值(P<0.01),而良性组增高不明显(P>0.05);以恶性组延迟(4h)断层显像T/TN值-x-1s为截断点,99mTc-HL91对乳腺癌诊断的灵敏度、特异度、准确度分别为93.55%(29/31)、80.00%(16/20)、88.24%(45/51),以良性组延迟(4h)断层显像T/TN值-x+1s为截断点,99mTc-HL91对乳腺癌诊断的灵敏度、特异度、准确度分别为93.75%(30/32)、84.21%(16/19)9、0.20%(46/51)。结论:99mTc-HL91乏氧显像对原发性乳腺癌的诊断具有较高的灵敏度和特异度,具有一定的临床应用价值,而且能提供乳腺癌的氧态信息,从而为制定治疗方案提供依据。 Objective:To evaluate the clinical value of ^99mTc-HL91 as a tumor positive imaging agent in diagnosis of breast neoplasm. Methods:Fifty-one patients with breast neoplasm underwent lh, 4h planar and 4h tomography fusion imaging after injection of ^99mTc-HL91. Regions of interest(ROI) were down on the tumor(T) and contralateral normal breast tissue(NT), T/NT raties were measured for all scans. Results:T/N ratios at 1h, 4h planar and 4h tomography imaging in malignant group were 1.68 ± 0.16,1.85 ± 0.23 and 2.28 ± 0.49,respectively;while in benign group, they were 1.15±0.18,1.23±0.25 and 1.43 ± 0.37,respectively ( P 〈0.01).4h delay T/N ratios in malignant group were obviously higher than lh ( P 〈 0.05), and there were no significant difference in benign group ( P 〉 0.05). Using malignant transaction T/N ratio( ^-x - Is) as diagnostic threshold, the sensitivity, specificity and accuracy were 93.55% ,80.00% and 88.24%, respectively. Using transaction T/N ratio( ^-x + Is) of benign group as diagnostic threshold, the sensitivity, specificity and accuracy were 93.75 %, 84.21% and 90.20%, respectively. Conclusion: The results indicate that as a tumor positive imaging agent ^99mTc-HL91 is useful in diagnesing breast carcinoma. T/NT value can provide oxygen status information of malignant lesions,so ^99mTc-HL91 can play an important role in optimizing the therapy for patients with breast cancer.
