摘要
目的探讨清晨1h尿白蛋白和肌酐(UAlb/Cr)的比值对早期糖尿病肾病的诊断价值。方法选取176例2型糖尿病患者,分别测定清晨1hUAlb/Cr、尿白蛋白排泄率及24h尿蛋白,同时用稳态模式胰岛素抵抗指数(HOMA-IR)估测外周组织胰岛素敏感性。对所有患者进行眼底检查,分析其相关性。结果清晨1hUAlb/Cr与尿白蛋白排泄率、24h尿蛋白以及HOMA-IR均具有显著正相关性,尤其是在早期糖尿病肾病阶段,且该比值与糖尿病视网膜病变具有良好的一致性。结论清晨1hUAlb/Cr可用于诊断早期糖尿病肾病,具有简便、迅速,受运动、体位等因素的影响小的优点。
Objective To investigate the application of urinary albumin/creatinine ratio（UAlb/Cr） in diagnosis of early diabetic nephropathy. Methods UAlb/Cr, urinary albumin excretion ratio and 24 h urinary total protein of 176 type 2 diabetic patients were detected. The tissue insulin sensitivity was assessed using formulae of homeostasis model assessment （HOMA-IR = （FPG x FINS）/22. 5）. The eyeground -scope examination was also carried out in the patients. Results The UAlb/Cr was positively correlated with urinary albumin excretion ratio, 24 h urinary total protein and HOMA-IR respecially in early diabetic nephropathy. UAlb/Cr had a good coincidence with diabetic retinopathy. Conclusion The UAlb/Cr can be used to diagnose early diabetic nephropathy, which is quick, convenient and not easily effected.
