摘要 目的:根据近年来国内外有关细胞移植治疗帕金森病的情况,探讨细胞替代性治疗的研究进展。资料来源:应用计算机检索Medline1994-01/2005-06相关细胞移植的文献,检索词“stemcell,braintransplantation”,并限定文献语言种类为English。同时计算机检索CBM2000-01/2005-06相关细胞移植方面的文献,检索词为“移植,帕金森病”,并限定语言种类为中文。资料选择:对资料进行初审,开始查找全文。纳入标准:选取包括细胞移植与帕金森病的文献。排除标准:综述文献、Meta分析、重复研究文章。资料提炼:将收集到的文章分为移植细胞的来源及脑内移植两部分进行评价分析。资料综合:目前临床上对帕金森病尚无有效的治疗办法,具有多分化能力的干细胞为帕金森病的替代性治疗带来了希望。细胞的来源较多,可以从胚胎干细胞诱导分化,可以直接提取神经干细胞,也可以利用骨髓基质干细胞等进行移植。移植的效果与移植技术关系密切,研究表明可以通过控制移植神经干细胞的数量、选择移植部位及利用支架材料等提高移植细胞的存活,提高治疗效果。结论:干细胞移植治疗帕金森病是可行的,经过深入的实验研究后可望从根本上治疗帕金森病。 OBJECTIVE: To investigate the progress of study on cell replacement on the basis of cell transplantation in treating Parkinson disease at home and abroad in recent years. DATA SOURCES: The English literatures about cell transplantation published between January 1994 and June 2005 were searched with computer in Medline database with the keywords of "stem cell, brain transplantation". Meanwhile, articles about cell transplantation published in Chinese between January 2000 and June 2005 were retrieved in CBM, the keywords were "transplantation, Parkinson disease" in Chinese. STUDY SELECTION: The literatures were checked firstly, and then the full-texts were searched. Inclusive criteria: the contents included cell transplantation and Parkinson disease. Exclusive criteria: review, Meta analysis and repetitive articles. DATA EXTRACTION: The literatures were divided into two parts of sources of transplanted cells and brain transplantation to assess and analyze. DATA SYNTHESIS: At present, there is no effective therapeutic method for Parkinson disease, stem cell replacement is a potential treatment to Parkinson disease. There are many kinds of cells for replacement material, such as embryonic stem cell, neural stem cell, bone marrow stromal cell etc. The effect of transplantation is closely correlated with the technique. The survival and differentiation were affected by transplantation techniques, such as the number of cells, position of transplantation, etc. CONCLUSION: Stem cell replacement is feasible to treat Parkinson disease. The deep research will give more directions to clinical work.
