摘要
对照研究了30例二尖瓣狭窄病例的连续性多普勒超声和三维可视化心音图资料,测量其二尖瓣口面积、跨瓣压、射流均速和峰速,及舒张期杂音时域能量、平均强度和最大强度,探讨舒张期杂音声学指标与二尖瓣局部解剖、血流动力学指标间的相关关系。结果表明,舒张期杂音能量和平均强度与瓣口面积呈负相关,与跨瓣压和射流速呈正相关,射流峰速和杂音最大强度呈正相关(0 .80 )。三维心音图能较好的量化分析杂音并可间接推断二尖瓣狭窄病变的程度。
To probe the relation between diastolic murmur and stenosis and haemodynamics, continuous Doppler ultrasonography with Visualized Phonocardiography(PCG) in 30 patients with mitral stenosis were compared. Mitral valve area, pressure and average and maximal velocity of shooting flow by echo and energy, average and maximal intensity of diastolic murmur were measured by PCG. The results showed that energy and average velocity of diastolic murmur were negatively related to mitral area and positively related to pressure and shooting velocity. Maximal shooting velocity is strongly related to maximal intensity (0.8). Visualized PCG can quantify murmur and deduce extent of mitral stenosis indirectly.
