血脂及血清载脂蛋白AⅠ、B测定对冠心病诊断的意义

The Significance of Diagnosing Coronary Heart Disease with Determing Plasma Lipoprotein and Apolipoprotein A1、B

目的:测定冠心病患者和健康人的血清总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)和载脂蛋白(apo)AⅠ、B及其比值,比较这些项目在两组间的差异,旨在研究载脂蛋白(apo)AⅠ、B比值与其余各项指标对冠心病的诊断价值。方法:随机选择冠心病患者40人,中位年龄62岁;健康人50人,中位年龄58岁,两组中性别无显著差异,用Olympus全自动生化分析仪分别测这两组的TC、TG、HDL-C、LDL-C、apoAⅠ、B。结果:用统计学方法处理所得结果,并做比较分析。发现冠心病组的TC、TG、LDL-C、apoB都明显高于健康人组(P<0.01),而HDL-C、apoAⅠ/apoB明显低于健康人组,且在各项目对冠心病的预测值有效率中,apoAⅠ/apoB的有效率最高(75.6%)。结论:apoAⅠ/apoB对于临床上诊断冠心病有重要价值。

Objective： To determine the plasma level of lipoprotein and apolipoprotein AⅠ ,B, and find out the difference of the plasma level of lipoprotein and apolipoprotein A Ⅰ , B in patients with coronary heart disease and healthy people. Methods： 40 patients with coronary heart disease and 50 healthy people were randomly divided into two groups,i, e. The CHD group （40 cases） and healthy group （50 eases）. The plasma level of lipoprotein and apolipoprotein A Ⅰ,B were determined in the two groups. Results： The plasma level of TC, TG,LDL-C and apolipoprotein B in CHD group were higher than that healthy group. The plasma level of HDL-C and the ratio of apolipoprotein A Ⅰ to B in CHDgroup were lower than that healthy group. There were significant difference between the two groups. Conclusion： The ratio between apolipoproteinA Ⅰ and apolipoproteinB（apoAⅠ/apoB） is very important for clinical diagnosis and prognosis of coronary heart disease.
