头部低温对大面积脑梗死高热患者血清皮质醇、超氧化物歧化酶、脂质过氧化物的影响 被引量:1

The effect of head hypothermia on the large-acreage cerebral insarction(LCI) and hyperthermia patient's serum CORT,SOD and LPO level

摘要 目的探讨大面积脑梗死(LCI)高热患者血清皮质醇(CORT)、脂质过氧化物(LPO)、超氧化物歧化酶(SOD)的含量变化,观察头部低温降温对三者的影响。方法采用放射免疫法、羟胺氧化法、丙二醛法测定49例(A组)LCI应用头部低温降温与43例(B组)应用温水擦浴、冰盐水灌肠降温者入院第2天及高热后第2天、第3天、第4天血清中CORT、LPO、SOD含量,并与30例健康人(C组)作对照。同时对两组生存者生活质量进行比较。结果A、B两组入院第2天较C组CORT、LPO明显升高和SOD降低(P<0.01)。A、B两组高热第2天较入院第2天CORT、LPO均明显升高和SOD显著降低(P<0.05,P<0.01)。A组高热第4天较入院第2天和高热第2天CORT、LPO显著降低(P<0.05,P<0.01),SOD升高(P<0.01)。B组高热第4天CORT、LPO仍高于和SOD低于入院第2天(P<0.05)。结论头部低温可有效使LCI高热患者CORT、LPO降低和SOD升高,提高预后生活质量。 Objective To study the changes of serum CORT,SOD and LPO in large-acreage cerebral insaretion(LCI) and hyl)erthermia patients,and observe the effect of head hypothcrmia on them. Methods All cases of LCI were divided into head hypothermia treatment group(group A, n = 49), warm water bath and glacial saline ene ma treatment group(group B, n= 43), and l,.eahh group (control group, group C, n = 30). With the methods of ra dioimmunoassay,hydroxylamine oxidation and thirddialdehyde, the serum CORT, SOD and LPO level in the two groups were measured on the 2nd day after hospitalization and on the 2nd day,the 3rd day and the 4th day after hy Derrhermia. In addition,the life quality scores of the two groups were compared. Results The serum CORT, LPO of group A and group B were higher on the 2nd day after hospitaiization than the group C(P〈 0.01 ) , while the SOD were lower(P 〈 0.01 ). The serum CORT, LPO of group A and group B were higher on the 2nd day after hyperthermia than on the 2nd day after hospitalization( P 〉 0.05 ), while the SOD were lower( P 〈 0.05, P 〈 0.01 ). The serum CORT and LPO of group A on the 4th day after hyperthermia were significantly lower than that of on the 2nd day after hyperthermia and after hospitalization( P 〈 0.05, P 〈 0.01 ), and the SOD level was significantly highcr than the latter( P 〈 0.01 ). The serum CORT of group B on the 4th day after hyperthermia was significantly lower than that of on the 2nd day ( P 〈 0.01 ). Conclusion Head hypothermia can effectively decrease the serum CORT, LPO level,and increase the SOD level of LCI and hyperthermia patients, and can enhance the prognosis quality of life as well.
