过敏性紫癜116例临床分析

Clinical Analysis of 116 Cases of Allergic Purpura

目的：分析过敏性紫癜发病的季节性、病因、诱因、内脏损害等临床特点，以提高该病诊断和治疗水平。方法：回顾性分析116例过敏性紫癜患者的临床资料，并予以统计分析。结果：62．1％的患者在12月～4月发病；39．7％患者发病前有上呼吸道感染；所有患者均有皮肤紫癜，腹痛者占51．7％，关节痛者占27．6％，出现肾脏损害者占44．8％，其中表现为肉眼血尿者占6．8％。在46例无明显诱因患者中，出现胃、十二指肠溃疡或炎症合并幽门螺杆菌（Hp）感染者12例，病毒感染7例。肾脏受累的程度决定患者的病程及预后。结论：应积极寻找过敏性紫癜的病因和诱因，特别注意上呼吸道感染、HP感染等，重视紫瘢性肾病（HSPN）的治疗。

Objective. To study the clinical features, diagnosis and therapy of Henoch-Sch？ nlein purpura （HSP）. Methods： 116 patients with H SP were retrospectively and statistically analyzed. Results. 62. 1% of patients with onset occurred between December to April next year. 39. 7% of the cases had a history of upper respiratory tract infection before the disease. All patients presented purpura, 51.7% with abdominal pain, 27. 6% joint pain, 44. 8% kidney damage, and 6. 8% with macroscopic hematuria. Among 46 cases without obvious inducement, 12 of them had peptic ulcer or inflammation complicating Helicobacter pylori（Hp） infection and 7 with vires infection. The degree of impairment of kidney damage determined the course of disease and prognosis. Conclusion. To look for the etiological and causative factors of HSP is important, especially the upper respiratory tract and Helicobacter pylori（Hp） infections. Rational treatment of HSP Nephritis （HSPN） should be emphasized.
