观察托拉塞米短期治疗充血性心力衰竭的疗效及对B型钠尿肽的影响

Observe the Effect of Torasemide Short-term Therapy in Patients with CHF and BNP

目的观察托拉塞米短期治疗充血性心力衰竭的疗效及对B型钠尿肽的影响。方法选取48名充血性心力衰竭患者,随机分为两组,一组给予托拉塞米,另一组为对照组(用速尿),治疗3天,记录两组患者治疗前后24 h尿量、体重、血压、血钾、射血分数(LVEF)、左室舒张末内径(LVEDD)及B型钠尿肽(BNP)的变化。结果治疗前,两组各参数无显著性差异;治疗3天后,托拉塞米组血压降低、EF值增加、左室舒张末内径缩小的幅度较对照组大,但两组间无显著性差异(P>0.05)。其它各变量托拉塞米组均较对照组改善显著(P<0.05),尤其是BNP下降明显(P<0.01)。托拉塞米组血钾轻度降低,但未出现低血钾,而对照组有6例低钾。结论短期应用托拉塞米可更快改善心功能,低血钾副作用轻,降低B型钠尿肽。

AIMObserve the effect of Torasemide short-term （ three days） therapy in patients with congestive heart failure（CHF） and brain natriuretic peptide （BNP）. METHODS Forty-eight patients with CHF were randomly divided into two groups： Torasemide-treated group and control group. Recorded urinary volume.body weight.blood pressure（BP） .serum potassium. LVEF. left ventricular endiastolic diameter （LVEDD） and BNP before and after therapy. RESULT There were significant differences on urinary volume.serum potassium and BNP between two groups after treatment. But no significant differences were observed about BP. weight.LVEF and LVEDD. No low serum potassium was observed in Torasemide-treated group but there were six patients with low serum potassium in control group after three days. CONCLUSION Torasemide short-term therapy in patients with CHF can low BNP and improve heart function rapidly than the other diuretic and incidence of hypokalemia is lower.
