Mirizzi综合征34例诊治分析

ANALYSIS OF DIAGNOSIS AND TREATMENT TO 34 CASES WITH MIRIZZI SYNDROME

目的探讨Mirizzi综合征的诊断与外科手术方法。方法回顾性分析1998年月1月-2004年12月,34例 Mirizzi综合征患者的临床资料,对其诊断及手术方式进行总结。结果按Csendes分型,Ⅰ型20例,Ⅱ型7例,Ⅲ型5例,Ⅳ型 2例。34例行胆囊切除、瘘口修补或胆肠Roux-en-Y吻合术。发生并发症2例,胆漏和肾功能不全各1例。结论Mirizzi综合征术前诊断困难,术中明确诊断及妥善手术处理是防止并发症的主要措施。

Objective To probe the methods of diagnosis and surgical operation for mirizzi syndrome. Methods To retrospectively analyze clinical datas of 34 cases with Mirizzi syndrome from January 1998 to December 2004,and make a summary to its diagnosis and operating methods. Result To differentiate the patterns of all patients by Csendes standard, pattern Ⅰ for 20 cases, pattern Ⅱ for 7 cacses, pattern Ⅲ for 5 cases, pattern Ⅳ for 2 cases. All 34 patients endured cholecystectomy, fistula repairment or biliointestinostomy by Roux-en-Y respectively. 2 cases occurred complications,biliary fistula and renal insufficiency for each respectively. Conclusion Its difficulty to make a confirmed diagnosis to Mirizzi syndrome before operation,aquiry a confirmed diagnosis and do everything properly in the course of operation is the principal measure to prevent from complication.
