结直肠癌血管内皮生长因子与螺旋CT灌注成像的相关性 被引量:6

Correlation between Vascular Endothelial Growth Factor and Spiral CT Perfusion Imaging in Colorectal Carcinoma

摘要 目的探讨结、直肠癌血管内皮生长因子(VEGF)与螺旋CT(SCT)灌注成像的相关性。资料与方法对30例结、直肠癌患者进行SCT单层动态增强扫描,绘制所选层面的癌灶、靶动脉的感兴趣区的时间-密度曲线(TDC)。根据TDC计算癌灶的血流灌注参数:强化峰值(PH)、对比剂峰值时间(PT)、强化比值(CER)、对比剂平均通过时间(MTT)、癌灶的灌注量(PF)。测定癌组织VEGF的表达。将结、直肠癌的VEGF的表达与各灌注参数进行比较,且两者亦分别与临床病理因素(Dukes分期、浆膜浸润及淋巴结转移)进行比较。结果结、直肠癌VEGF表达阳性率为87%(26/30),其与各灌注参数间均无显著相关性(P值均>0.05);VEGF、各灌注参数在Dukes分期、浆膜浸润、淋巴结转移比较中差异均无统计学意义。结论VEGF与SCT灌注成像均能反映结、直肠癌的血管生成状况,但两者之间无明显相关性。 Objective To evaluate the correlation between vascular endothelial growth factor (VEGF) and spiral CT perfusion imaging in colorectal carcinoma. Materials and Methods Dynamic contrast enhanced spiral CT was performed in 30 patients with pathologically verified eolorectal carcinoma. Time density curves (TDC) were created from the region of interest drawn over the tumor, target artery by spiral CT with perfusion functional software. The parameters were measured including: peak height (PH), peak time (PT), mean transit time (MTY), contrast enhancement ratio (CER) and perfusion (PF). VEGF was evaluated by using immunohistochemical staining of surgical specimens with anti VEGF monoclonal antibody, and then VEGF was compared with parameters of spiral CT perfusion. The correlation between VEGF, the parameters and clinicopathological factors (Dukes' stages, depth of invasion and lymph node metastasis) was also studied. Results The positive rate of VEGF expression was 87%, there was no significant correlation between the VEGF expression and the parameters of spiral CT perfusion (P 〉 0.05). VEGF and the perfusion parameters were not associated with invasive depth, metastasis and disease stage. Conclusion Both VEGF and spiral CT perfusion imaging can reflect angiogenic activity, but no significant correlation is found between them.
