摘要
目的通过对胃癌组织中MRP1mRNA,MRP2mRNA的表达水平及MDR基因产物的检测,进一步了解肿瘤多药耐药机制与胃癌发生发展、生物学行为和预后的关系。方法胃癌组织中MRP1mRNA,MRP2mRNA的表达采用原位杂交染色法。MDR基因产物的检测应用SP免疫组织化学法。结果67例胃癌组织中MRP1mRNA和MRP2mRNA表达阳性率分别为68.7%和71.6%,高分化腺癌和区域淋巴结未转移癌中MRP1mRNA和MRP2mRNA表达阳性率明显高于未分化腺癌和伴淋巴结转移癌(P<0.05,P<0.01),而且不同组织类型中MDR基因产物P170(P-gp),GS T-π及TOPO-Ⅱ的表达不同,有淋巴结转移的胃癌患者P-gp的表达较高,而GS T-π及TOPO-Ⅱ的表达与有无淋巴结转移无关。结论MRP1mRNA和MRP2mR-NA的表达水平与MDR基因产物的分配有密切关系,检测胃癌组织中多药耐药相关蛋白mRNA表达的同时检测MDR基因产物对了解胃癌的发生发展、生物学行为和预后,以及指导化疗均具有重要的临床意义。
Objective To interpret the relationship between the mechenism of multidrug resistance (MDR) and the onset, developing , biological behavior as well as prognosis of gastric cancer by investigating the expression level of MRP1 mRNA, MRP2 mRNA and MDR gene product in tissue of gastric cancer. Methods The expression of MRP1 mRNA , MRP2 mRNA and MDR gene product in the gastric cancer were detected by Hybridization in Situ and SP immunohistochemistry respectively. Results The positive rates of MRP1 mRNA and MRP2 mRNA were 68.7% and 71.6% in 67 cases of gastric cancer,The positive rates of MRP1 mRNA and MRP2 mRNA were significantly higher in the cases of well - differentiation and without lymphnode metastasis than that in the patients of undifferentiation and without metastasis( P 〈0.05 , P 〈 0.01 ). The expression of MDR gene product such as P170 ( P - gp ) , GS T - πand TOPO - Ⅱ were different with different pathology category, the expression of P - gp was significantly higher in the cases of lymphnode metastasis than others ,but the expression of GS T - πand TOPO - Ⅱ were no relationship to lymphnode metastasis. Conclusion The expression levels of MRP1 mRNA and MRP2 mRNA have a close relationship to the distribution of MDR gene product. It have an very important clinical meaning not only to know the onset, developing , biological behavior and prognosis of gastric cancer but also to guide the chemotherapy by detection of the expressions of multidrag resistance - associated proteins mRNA and MDR gene product simultaneionsly.
