摘要
目的探讨P-gp、PKC-α和MRP在上皮性卵巢癌中的表达及其临床意义。方法运用免疫组织化学技术(S-P法)检测P-gp、PKC-α和MRP在50例上皮性卵巢癌、20例卵巢良性肿瘤和10例正常卵巢组织中的表达。结果P-gp、PKC-α和MRP三个指标在上皮性卵巢癌组织中阳性率分别为48.0%、56.0%、60.0%,显著高于正常卵巢和良性肿瘤(P<0.05);P-gp和PKC-α表达存在密切的相关关系(P<0.01);卵巢癌P-gp、PKC-α和MRP表达阳性者的化疗反应率分别为25.0%、25.0%、33.3%,低于阴性表达组(P<0.05);复发患者P-gp和PKC-α阳性表达率明显高于未复发的患者(P<0.05),MRP表达与复发无关(P>0.05)。单因素分析表明,P-gp、PKC-α和MRP与卵巢癌患者的生存期和不良预后有关。多因素分析表明,P-gp及三个指标共同表达与患者的生存率有关。结论卵巢恶性肿瘤中P-gp、PKC-α和MRP阳性表达与临床化疗耐药有关,并影响患者的生存率。联合检测三个指标对临床化疗疗效判断和患者预后估计具有积极的指导作用。
Objective To investigate the expression of P - gp, PKC -α and MRP in epithial ovarian carcinoma and its significance. Methods Expression of P - gp,PKC -α and MRP examined in 50 epithelial ovarian carcinoma by means of immunohistochemisty ( method of S - P ) . Results The positive expressions of P - gp, PKC -α and MRP in patients with ovarian carcinoma were 48.0% ,56.0% ,60.0% respectively and all higher than those in benign tumors and normal controls( P 〈0.05 ) ; the expression of P - gp had correlation with PKC -α ( P 〈 0. 01) ; the response rates to chemotherapy of the P - gp,PKC -α and MRP positive group were 25.0% ,25.0% ,33.3% repectively , significantly lower than that of the P- gp,PKC -α and MRP negative group(P 〈0.05) ;the expression of P - gp and PKC -α were more frequently detected in recurrent tumors ( P 〈 0. 05 ). while the expression of MRP was not related to recurrence ( P 〉0.05 ). Univariate analysis showed that the expression of P - gp,PKC -α and MRP was correlated with survival time and poor prognosis of patiens with ovarian carcinoma . Cox regression analysis showed that the expression of P -gp and co -expression of P -gp,PKC -α and MRP was correlated with survival rate. Conclution The over - expression of P - gp,PKC -α and MRP is correlated with drug resistance of clinical chemotherapy and survival rate in ovarian cancer. To examine these 3 indexes in ovarian cancer is helpful to judge reponse to clinical chemotherapy and to evaluate the prognosis.
