摘要
目的探讨心电图Ⅰ导联对左侧气胸的诊断价值。方法对比分析60例坐位与卧位Ⅰ和V6导联R波、S波电压和R/S值(与心电向量图X轴R/S值对照),及坐位时R波、S波的增长率。结果Ⅰ与V6导联检测结果相同,两者增长率差异无显著性意义(P>0.05),R/S值Ⅰ导联卧、坐位分别为2.38±1.75、3.60±1.46;V6导联卧、坐位分别为4.62±2.69、6.50±3.12,两个导联差异均有显著性意义(P均<0.05)。X轴R/S值卧位为2.50±1.89,坐位为3.10±1.68,与Ⅰ导联差异无显著性意义(P>0.05),但与V6导联差异有显著性意义(P<0.05)。结论Ⅰ导联诊断左侧气胸价值优于V6导联,Ⅰ、V6导联联合应用能更好地提高诊断正确性。
Objective To evaluate significance of lead Ⅰ on electrocardiogram (ECG) in diagnosis of left pneumothorax. Methods Amplitudes of R wave, S wave and R / S in leads Ⅰ and V6 of ECG recorded in sitting and supine positions were analyzed. Results Amplitude of R and S waves in lead Ⅰ was similar to that in lead Vs (P〉0.05). R / S in lead Ⅰ was 2.38±1.75 in supine position and 3.60±1.46 in sitting position and similar to R / S in X axis of vectorcardiogram (2.50± 1.89 and 3.10±1.68) recorded in corresponding positions. While R/S in lead V6 was 4.62±2.69 in supine position and 6.50±3.12 in sitting position and they were significantly higher than those in lead Ⅰ (P〈0.05) and those of vectorcardiogram. Conclusion Lead Ⅰ is superior to lead V6 in diagnosis of left pneumothorax based on ECG. combination with lead Ⅰ and lead V6 will inprove correctness.
