超声对消化道溃疡穿孔的诊断价值

The value of ultrasonography in diagnosing ulcerous perforation of digestive tract

目的探讨超声对消化道溃疡穿孔的诊断价值。方法对33例经手术证实为消化道穿孔患者的超声检查结果与X线检查结果进行回顾性对比分析。结果33例患者中有27例的腹腔游离气体被超声和X线正确诊断,符合率为82%。29例患者超声诊断为腹腔积液,4例包裹性消化道穿孔超声和X线均未发现游离气体,其中3例超声发现了肿块和包裹性积液。结论在发现腹腔少量游离气体方面超声可以与X线检查相媲美,且超声还可发现腹盆腔积液、肿块等X线不能发现的并发症,弥补了X线的不足,是消化道溃疡穿孔的一项有效诊断方法。

Objective To explore the diagnostic value of ultrasonography for the ulcerous perforation of digestive tract. Methods Both ultrasonography and radiography were carried out in 33 cases with ulcerous perforation of digestive trace,and the result were comparably analysed. Results Of these 33 cases, free air was finded in 27patients by both uhrasonography and radiography, and the agreement rate was 82%. Ultrasonography had affirmed that 29 cases had ascites. Both uhrasonography and radiography did not find free air in 4 patients with encapsulated perforation of digestive tract, but ultrasonography showed masses and encapsulated ascites. Conclusion Uhrasonography is as the same sensitive as radiography in detecting a little free air in abdomihal cavity Furthermore, uhrasonography can detect some other complications of ulcerous perforation of digestive trace such as ascites, masses abdomen,which usually can not be found by X - ray,and ultrasonography could made up the defect of radiography,so it is an practical method in diagnosing ulcerous perforation of digestive tract.
