应用胰岛素泵与皮下注射胰岛素治疗糖尿病临床效果研究

Insulin Pumps and Subcutaneous Injection of Insulin in Treatment of Diabetes Mellitus: Clinically Comparative Study

目的:比较常规皮下注射胰岛素(MSII)与胰岛素泵(CSII)治疗2型糖尿病效果及安全性。方法:将1 652例糖尿病随机分为两组,CSII组827例采用不同类型的胰岛素泵进行连续皮下胰岛素注射,MSII组825例采用多次皮下胰岛素注射,比较两种治疗方法在控制血糖、胰岛素剂量、低血糖发生率方面的差别。结果:CSII组达到理想血糖控制效果所需时间明显优于MSII组(P<0.01),达到理想血糖控制效果所需胰岛素剂量明显少于MSII组(P<0.05),低血糖发生率也低于MSII组(P<0.01)。结论:CSII治疗能够缩短达到理想血糖控制所需时间,有效降低餐后2小时血糖,减少胰岛素用量,减少低血糖的发生。

Objective：To compare the therapeutic effects and safety of continuous subcutaneous insulin infusion by insulin pump （CSⅡ） and multiple subcutaneous insulin injection （MSⅡ） in treatment of type 2 diabetes mellitus. Methods：Totally, 1652 cases of diabetes mellitus were randomly assigned into two groups, CSⅡ group （ n = 827 ） and MSⅡ group （ n = 825 ）. The differences in blood glucose control time, insulin dosage and the rate of hypoglycemic episodes were compared in the two groups. Results：The time for well controlled blood glucose levels in CSⅡ group was significantly shorter than that in MSⅡ group （P 〈 0.01 ） ,the insulin dosage needed for well controlled blood glucose level in CSⅡ group was obviously less than that in MSⅡ group （P〈0. 05 ）, hypoglycemic episode of CSⅡ group was also lower than that of MSⅡ group （P 〈 0. 01 ）. Conclusions ：CSⅡ could shorten the duration of insulin treatment,lower 2-hour postprandial blood sugar （PBS2） and reduce insulin dosage,and hypoglycemic episodes.
