脑保护滤网在颈动脉支架成形术中的应用

Application of cerebral protection filter in carotid angioplasty and stenting

目的观察脑保护滤网在颈动脉支架成形术中的疗效和安全性。方法12例症状性颈动脉狭窄患者行血管内支架成形术时使用脑保护滤网，观察脑保护滤网中有无脱落的斑块以及围手术期有无并发症。术后B超随访观察有无血管再狭窄以及有无缺血性脑血管事件。结果12例患者脑保护滤网放景和回收以及支架放置全部成功，其中7例在回收的脑保护滤网中发现有斑块碎片，无围手术期并发症，随访1-8（4．89±2．67）个月，原狭窄部位血管通畅，未见短暂性脑缺血发作或脑梗死再发。结论使用脑保护滤网可提高颈动脉支架成形术的安全性。

Objective To observe the effect and safety of cerebral protection filters in the prevention of cerebral embolic events during carotid angioplasty and stenting. Methods Twelve patients with symptomatic carotid artery stenoses were treated with carotid angioplasty and stenting using cerebral protectin filters. The incidences of the dislocated debris found in the filters and perioperative complications were noted. The stented artery restenosis and cerebral ischemic events were followedup. Results The filter devices were successfully placed and retrieved and the all earotid stents were successfully implanted on the first procedure without procedural and periprocedural complications in all 12 patients. Plaque debrises were found with naked eye in 7 of 12 retrieved filters ,During the follow-up period of 1 -8 （4. 89 ±2.67） months, there was no transient ischaemic attack or cerebral infarction and all stented vessels remained patent. Conclusion Use of filter device may improve the safety of carotid angioplasty and stenting.
