经小切口直接外侧入路行全髋置换术

Total hip replacement through a short direct lateral approach

目的总结经小切口直接外侧入路行全髋置换术(THR)的方法和疗效。方法从2002年2月至2004年2月,采用小切口直接外侧入路手术65例,70髋。随访至少满半年。结果手术切口平均长8.8 cm,术中出血量平均250 ml,手术时间平均65 min,术后24h引流量平均200ml。术后疼痛程度和持续时间均较常规入路显著改善。术后X线片显示假体位置良好。Harris评分平均98分。结论该入路具有暴露好,创伤小,假体安装定位准确,功能恢复快等优点。

Objective To summarize the way and effect of total hip replacement through the short direct lateral approach. Methods From February 2002 to February 2004, 65 cases （70 hips） undervent total hip replacement with the short direct lateral approach. The time of follow-up was above 6 months. Results The average length of skin incision was 8.8 cm. The average of operation time was 65 minutes. The patients had an interoperative blood loss of 250 ml and a drainage of 200 rnl within 24 hours after operation with less postoperative pain. Radiographs showed good location of implants. The average Harris hip score was 98 points. Conclusion The short direct lateral approach has advantages of less injury,well exposure,accurate location of prosthesis and early recovery.
