小切口辅助后腹腔镜下肾切除术

Small incision-assisted retroperitoneal laparoscopic nephrectomy

目的探讨小切口辅助后腹腔镜下肾切除手术的方法。方法施行了10例小切口辅助后腹腔镜下肾切除手术，包括肾癌根治术6例，重度肾积水2例，肾结核2例。采用健侧卧位，经腋中线第12肋缘下切4cm左右的小切口，直视结合腹腔镜下处理肾蒂，然后将肾脏经肋缘下小切口取出。结果手术均获成功，手术时间90～192min，均未输血，未发生任何手术并发症。结论与腹腔镜肾切除术相比，这种方法无需充气，使用普通的手术器械，更接近常规手术操作，降低手术费用，操作方便。

Objective To summarize the technique of small incision-assisted retroperitoneal laparoseopie nephreetomy （SIREN）, Methods SIRLN was performed in 10 pationts （6 eases of radieal nephrectomy,2 eases of severe hydronephrosis, and 2 eases of renal tuberculosis）, The patient was positioned in a normal lateral decubitus position. About 4 cm-long incision was made below the twelfth costal margin through midaxillary line. The injured kidney was taken out by laparoseope through the small cut. Results The operations were successfully completed without serious complications. The operation tooke 90-192 minutes with blood loss of 20-105 ml. Conclusion The technique does not need retroperitoneal inflation. The operation is performed easily with common surgical instruments and less cost.
