冠状动脉旁路移植术中骨骼肌化胸廓内动脉的应用

Skeletonized internal thoracic artery harvesting incoronary artery bypass grafting

目的 探讨骨骼肌化胸廓内动脉（ITA）用于冠状动脉旁路移植术的优越性。方法采用剪刀和钛夹将骨骼肌化的ITA游离，并将其用于冠状动脉旁路移植术。结果共游离ITA 61根，并顺利用于60例冠状动脉旁路移植术患者，平均获取时间为26．8min。骨骼肌化ITA平均血流量吻合前为86．5ml／min，吻合后为26.4ml／min。随访1～31个月，无心脏相关性死亡和事件发生。结论在冠状动脉旁路移植术中应用骨骼肌化的ITA和带蒂ITA一样安全、有效。

Objective：To evaluate the advantages of harvesting skeletonized internal thoracic artery（ITA） in coronary artery bypass grafting. Methods ：The internal thoracic artery was mobilized from the satellite veins and surrounding tissue mainly using scissors and clips in 60 patients underwent coronary artery bypass grafting. Results： 61 skeletonized ITAs were mobilized, the mean time of harvesting was 26.8 min. The mean blood flow of skeletonized ITA before anastomosis was 86.5ml/min,after anastomosis was 26.4ml/min. Followup data showed there were no cardiac deaths or cardiac events. Conclusion： Skeletonized ITA is as safe and effective as ITA with pedicel in coronary artery bypass grafting.
