摘要
[目的]观察胃癌细胞p27kip1、cyclinE的表达情况,并分析幽门螺杆菌(Helicobacter pylori,Hp)感染对二者表达的影响,以探讨Hp可能的致癌机制。[方法]用免疫组织化学SP法检测59例胃癌p27kip1、cyclinE表达情况,采用改良Giemsa快速尿素酶检验法检测Hp感染。[结果]p27kip1阳性率在正常胃黏膜为84.22%,显著高于胃癌的47.46%(P<0.05),胃癌中高中分化腺癌组为68.42%,显著高于低分化腺癌的37.5%和与胃黏液癌的31.25%(P<0.05),但后两者间无显著性差异(P>0.05)。cyclinE阳性率在正常胃粘膜为11.11%,显著低于胃癌的55.93%(P<0.05),胃癌中高中分化腺癌组为31.58%显著低于低分化腺癌的66.67%和胃黏液癌的68.75%(P<0.05),后两者间无显著性差异(P>0.05)。p27kip1及cyclinE的阳性率在Hp阳性和Hp阴性胃癌组间无显著差异性(P>0.05)。[结论]胃癌组织p27kip1表达下调、cyclinE表达上调,提示二者的表达失衡可能会促进胃癌的发生。但二者表达与Hp感染无明显相关关系。
[ Objective] To investigate p27^kip1 and cyclinE gene product expressions of carcinoma cells and the relationship of the expressions to Helicobacter pylori (Hp) infection in gastric carcinoma. [ Methods] The expressions of p27^kip1 and cyclinE in 59 gastric carcinomas were detected by immunohistochemical method (SP method) and Hp was examined by Giemsa staining and rapid urease test. [ Results] The p27^kip1 expression rate of normal gastric mucosa being 84.22% was significantly higher than that of gastric carcinoma being 47.46% (P 〈 0.05); the rate of well- middle differentiative carcinomas being 68.42% was significantly higher than those of poor differentiative carcinoma being 37.50% and mucoid carcinoma being 31.25% (P 〈 0.05) . The cyclinE expression rate of normal gastric mucosa being 11.11% was significantly lower than that of gastric carcinoma being 55.93% (P 〈 0.05); the rate of well- middle differentiative carcinomas being 31.58% was significantly lower than those of poor differentiative carcinoma being 66.67% and mucoid carcinoma being 68.75% ( P 〈 0.05). In gastric carcinoma, there was no significant relationship of the expressions of p27^kip1 and cyclinE to Hp infection ( P 〉 0.05). [ Conclusion] The lower expression of p27^kip1 and the higher expression of cyclinE are associated with the development and the progression of gastric cancer, and there was no singificant association of the expression of p27^kip1/cyclinE with Hp infection.
