窄谱与宽谱中波紫外线治疗银屑病的临床疗效分析 被引量:19

Analysis of clinical efficacy of 311 nm narrow band UVB and 280~320nm broad band UVB phototherapy in chronic plaque psoriasis

摘要 目的:观察窄谱(311nm)中波紫外线(NB-UVB)与宽谱(280~320nm)中波紫外线(BB-UVB)治疗慢性斑块状银屑病的临床疗效。方法:采用自身双侧对比照射的方法,分别照射NB-UVB和BB-UVB治疗50例银屑病患者,并以银屑病严重程度(ESI)评分标准评分,分析比较NB-UVB和BB-UVB治疗银屑病的疗效。结果:治疗1周后,NB-UVB组患者的ESI评分下降(P<0.05),BB-UVB组患者ESI评分无明显改善(P>0.05)。皮损清除率NB-UVB组显著高于BB-UVB组(P<0.05),两组治愈率相比差异无统计学意义(P>0.05);治疗2、3、4周与治疗1周时相比,NB-UVB组ESI评分低于BB-UVB组(P<0.05),皮损清除率和治愈率高于BB-UVB组(P<0.05)。结论:NB-UVB治疗慢性斑块状银屑病疗程短,治愈率高,其临床疗效明显优于BB-UVB。 Objective: To observe and to analyze the clinical efficacy of 311 nm narrow band UVB (NB-UVB) and 280-320 nm broad band UVB (BB-UVB) phototherapy in chronic plaque psoriasis. Methods: A total of 50 patients with chronic plaque psoriasis were treated by 311 nm NB-UVB radiation at left part of body and 280-320nm UVB radiation at right part of body. The efficacy was evaluated by ESI score, and analysis was performed by comparison of efficacy of NB-UVB and BB-UVB. Results: After 1 week of treatment, ESI scores of NB-UVB group decreased significantly(P〈 0.05), while which of BB-UVB had not significant difference(P〉 0.05). ESI scores of NB-UVB decreased more significantly than those of BB-UVB(P 〈 0.05), and clearance rates of lesions of NB-UVB increased more significantly (P 〈 0.05), between NB-UVB and BB-UVB group no significant difference of cure rates was showed (P〉0.05). In the 4th week, as compared with the 1st week, ESI scores of NB-UVB decreased more significantly than those of BB-UVB(P〈0.05), and clearance rates of lesions and cure rates of NB-UVB as compared with BB-UVB group increased more significanfly(P〈0.05). Conclusion: In contrast to BB-UVB, NB-UVB has the advantages of shorter period, higher cure rates and superior clinical efficacy for chronic plaque psoriasis.
