肝脏炎性假瘤的超声造影表现

Contrast-enhanced ultrasonographic features of inflammatory pseudotumor of the liver

目的观察肝脏炎性假瘤超声造影的表现,探讨超声造影对肝炎性假瘤的诊断价值。方法经外周静脉团注声诺维,使用对比脉冲序列(CPS)技术对2例经超声引导下活检证实的肝脏炎性假瘤行超声造影检查,观察注射造影剂后病灶的增强情况。结果2例炎性假瘤均表现为动脉期迅速增强并快速廓清,门脉期与实质期回声显著低于周边肝实质。结论肝脏炎性假瘤的超声造影灌注特征可以与肝脏恶性肿瘤相类似,造影诊断为恶性的病灶并不能完全除外炎性假瘤的可能,应行超声引导下活检以明确病理诊断。

Objective To investigate the perfusion features of hepatic inflammatory pscudotumor on contrast-enhanced ultrasound. Methods With bolus injection of SonoVue via peripheral vein and the imaging technique of contrast pulse sequence （ CPS）, two patients with biopsy-proved hepatic inflammatory pseudotumor were examined by contrast-enhanced ultrasound. The enhancement patterns of the lesions were observed. Results Both lesions showed an early, rapid wash-in and early, rapid wash-out pattern in the arterial phase. At the portal phase and parenchyma phase their echogenicity were significantly lower than that of peripheral liver parenchyma. Conclusions Hepatic inflammatory pseudotumor may show an enhancement pattern similar to that of liver malignancy. As the possibility of inflammatory pseudotumor cannot be ruled out completely if the lesion is suspected to be malignant on contrast-enhanced ultrasound, in some cases ultrasound-guided biopsy should be performed to obtain definite pathologic diagnosis.
